Our commitment to your privacy

Seva Home Care Illinois, LLC (“Seva,” “we,” “us,” or “our”) is committed to protecting the privacy of your health information. We are required by law to maintain the privacy of protected health information (“PHI”), to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we may use and disclose your health information

We may use and disclose your PHI for the following purposes:

• Treatment and care. To provide, coordinate, and manage your care, for example, sharing information among our caregivers and with other providers involved in your care.
• Payment. To bill and obtain payment for services from you, an insurer, a long-term care policy, or a program such as VA benefits.
• Health care operations. For activities such as quality review, training, scheduling, licensing, and general administration.
• As required by law. When federal, state, or local law requires it.
• Health and safety. To prevent a serious threat to your health and safety or that of others.
• Public health and oversight. To public health authorities or oversight agencies as permitted by law.
• Family and others involved in your care. With people you identify as involved in your care, to the extent permitted and consistent with your wishes.

Uses and disclosures that require your authorization

Other uses and disclosures, including most uses of psychotherapy notes, marketing, and any sale of PHI, will be made only with your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already relied on it.

Your rights regarding your health information

• Access. You may request to inspect and receive a copy of your health information we maintain.
• Amendment. You may request that we correct information you believe is incorrect or incomplete.
• Accounting of disclosures. You may request a list of certain disclosures we have made.
• Restrictions. You may request limits on how we use or disclose your information; we will accommodate reasonable requests where required or feasible.
• Confidential communications. You may request that we contact you in a specific way or at a specific location.
• Paper copy. You may request a paper copy of this notice at any time.
• Notice of breach. You have the right to be notified if there is a breach of your unsecured PHI.

Our responsibilities

• We are required to maintain the privacy and security of your PHI;
• We will let you know promptly if a breach occurs that may have compromised your information;
• We must follow the duties and privacy practices described in this notice and give you a copy of it; and
• We will not use or share your information other than as described here unless you tell us we can in writing.

Changes to this notice

We may change this notice and make the new terms effective for all information we maintain. The current notice will be posted on our website and available upon request, with the effective date shown above.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
